FastMed MyChart Terms & Conditions of Use FastMed is pleased to offer our patients online computer access to portions of their medical record through the MyChart web portal. If you believe that you would benefit from this service, you may sign up for access by completing the MyChart registration process and agreeing to these terms and conditions. Use of MyChart I understand that MyChart should NEVER be used for urgent health care matters. The anticipated turnaround time for a response to a secure message is two business days. Therefore, if I believe I am experiencing an urgent or life-threatening health condition I should promptly visit a FastMed urgent care clinic for an in-person evaluation, go directly to the Emergency Department of a local hospital, or call 911 as appropriate depending on the severity of distress. If your medical provider is out of the office or unavailable to respond to your secure email message, your message may be routed to another FastMed provider. At this time, we anticipate that the MyChart web portal will provide the following services: Ability to review, make, or cancel appointments Ability to view selected portions of your FastMed medical record such as allergies, medications, histories, problem list, and recent visit information Ability to receive selected lab results Communicate with your physician via secure messaging regarding health concerns A library of searchable general health information Other services may be added as FastMed implements the MyChart web portal. I understand that FastMed providers may send me messages via MyChart. It is my responsibility to monitor these messages and respond in an appropriate manner. I must provide FastMed with a valid email address and update FastMed if I change my email account. FastMed may send me emails via my personal email address to notify me that I have new MyChart messages. Messages sent via my personal email will not contain personal health information. I agree that all messages that are sent through MyChart will be regarding MY health care and conditions. I understand that asking for advice for a friend or family member is not allowed with a limited exception if you are an authorized caregiver and MyChart proxy user for a FastMed patient. Since these messages may become a part of my permanent medical record and will be perceived as my health information, it is important that no one else’s information be reflected in my messages. With my authorization, this information may be released to insurance companies when applying for insurance. FastMed takes no responsibility for erroneous medical information that you provide via MyChart that may impact your ability to be covered by insurance. Without my authorization, medical information may be released to my insurance for payment of my health care claims. Use of MyChart will be governed by Delaware law. Confidentiality Both Federal and relevant State laws govern the confidentiality of your medical information. FastMed is dedicated to keeping my medical information confidential. Our employees are subject to strict standards for maintaining your confidentiality. By allowing you access to your medical information you must share this responsibility. Please read FastMed’s Notice of Privacy Practices for more information. Personal Identification Code and Password I understand that I will be given a unique access code by FastMed to allow me access to MyChart. The first time I log into MyChart, I will be required to create a unique six-character User ID and a six-character password. I agree to keep my user ID and password confidential. Anyone with access to my user ID and password would have access to my personal health information, ability to add to my personal health information, and message my providers as if they are me. It is my responsibility to change my password if I believe it has been compromised or shared with someone else. It is recommended that you change your password every 180 days. Every time I log into MyChart, my user ID and password will be logged in an audit trail in the computer system. I understand that if FastMed discovers that I have inappropriately shared my password or that I have abused my access privileges, FastMed may terminate my participation in MyChart. I understand that FastMed takes no responsibility for and disclaims any and all liability for damages arising from a breach of medical record confidentiality resulting from my sharing or losing my password. Availability I understand that MyChart may not always be available to me due to system maintenance, back-ups, or failures. Access is provided on an “as is available” basis and I am not guaranteed access at any particular time. Security FastMed has implemented the following security features: Encryption: We use 128-bit Secure Socket Layer to encrypt the communication from your web browser to our secure web server. Dedicated web server for MyChart: We separate the web server and electronic medical record machines. In the unlikely event that our server is compromised, no health information would be exposed since it is not saved on the web server. Patient information is stored only in the electronic medical record, behind a firewall. No Caching on the client side: We disable caching of data in the browser and force the web pages to expire immediately. This prevents a follow-on user from viewing pages that you had previously viewed while using MyChart. Firewall: Personal health information is stored in the secure electronic medical record, protected from the Internet by a firewall. Session Timeout: After a period of inactivity, the logon session will time out. Monitoring: We constantly monitor the web server for evidence of unauthorized break-in attempts. We apply the latest security patches, as they become available.


You may choose to use other services provided by third parties in conjunction with your use of MyChart. Such services may include, but are not limited to, translation services, location services, and other third-party offerings. Any links to such services are provided for your convenience only. Neither Epic nor FastMed have control over the contents of these services, and neither Epic nor FastMed accept any responsibility for them or for any loss or damage that may arise from your use of them. Such services may require you to submit your information to a third party, which may result in a copy of your information being used and/or stored by a third party. If you decide to access any such services in conjunction with your use of MyChart, you do so entirely at your own risk and subject to the terms and conditions of use for such services.
